Thank you for your interest in becoming Certified SHEmakes! We are so excited you would like to join us, and we look forward to reviewing your application materials. To complete the full application for consideration to be SHEmakes certified, you must submit all required information on this form, as well as a completed payment of the full application fee  

(current Innovator price set at $499 + applicable tax). If you have not prepared any materials, or are not familiar with what materials are needed, please refer to the SHE Program           and Frequently Asked Questions         .

What is the company's average rate of employee turnover for the last 12 months?*

To calculate, follow this formula:

  1. Pick a period of time that you want to measure your employee turnover rate over - for the purposes of this application, that is 12 months.

  2. Add the number of employees at the beginning of the time period to the number of  employees at the end of the time period. Then, divide that number by two. This quotient will be your average number of employees.

  3. Divide the number of employees who left during this time period by the average number of employees.

  4. Multiply this quotient by 100 to calculate the turnover rate.
